According to the Budget Code Russian Federation and Federal Law No. 161-FZ of November 14, 2002 "On State and Municipal unitary enterprises": 1. To increase in 2011 the authorized capital of the State Unitary Enterprise of the city of Moscow "Moscow Order of Lenin and the Order of the Red Banner of Labor Metro named after V.I. Lenin" (GUP "Moscow Metro") by 11,200,000,000 rubles , including: - 8,500,000,000 rubles for the purpose of investing in capital construction facilities of the Moscow metro in accordance with the annex to this order; - 2,700,000,000 rubles for purposes in accordance with the feasibility study for increasing the authorized capital of the State Unitary Enterprise " Moscow Metro" (hereinafter - Feasibility Study). 2. Establish that in order to increase the authorized capital of SUE "Moscow Metro" (clause 1), the following conditions must be met: 2.1. Submission within a week from the date of entry into force of this order together with the approved feasibility study for the amount of 11,200,000,000 rubles (clause 1) indicating the purposes, volumes and periods of spending the funds provided from the budget of the city of Moscow. the moment of economic policy and development of the city of Moscow, examination of the feasibility study (clause 2.1) within two weeks from the date of its submission. 3. Establish that measures to increase the authorized capital of the State Unitary Enterprise "Moscow Metro" are carried out by the Department of Property of the City of Moscow, subject to a positive conclusion on the feasibility study (clause 2.2). 4. Establish that the transactions specified in the feasibility study (clause 2.1) related to investing in the capital construction of metro facilities are agreed upon by the owner of the property in accordance with the provisions of Article 23 federal law dated November 14, 2002 N 161-FZ "On State and Municipal Unitary Enterprises". 5. Together with the Department of Transport and Development of Road Infrastructure of the City of Moscow, ensure quarterly submission to the Department of Property of the City of Moscow and reports of the State Unitary Enterprise "Moscow Metro" on the implementation of the feasibility study, indicating the purposes of spending the provided budget funds, volumes of their spending and achievement of planned indicators of the feasibility study. 6. Recognize invalid the order of the Government of Moscow dated "On increasing the authorized capital of the State Unitary Enterprise "Moscow Metro" in 2011". 7. To impose control over the implementation of this order on the Deputy Mayor of Moscow, the head Sergunina N.A. and Deputy Mayor of Moscow in Khusnullin M.Sh. Moscow Mayor S.S. 2.2. the report in form N 5 "Information on preventive vaccinations" includes:
Information about completed vaccinations only. For example, if a vaccination involves four vaccinations, then only those who were vaccinated four times would be included in the report;
Information about all revaccinations;
Information about vaccinations carried out with vaccines (anatoxins) of domestic or foreign production, registered and authorized for use in the prescribed manner;
Information about vaccinations carried out within the framework of the national calendar of preventive vaccinations, the calendar of preventive vaccinations according to epidemic indications and additional immunization within the framework of the priority national project;
2.3. line 01 "vaccination against whooping cough" includes the number of children who received three doses of pertussis, diphtheria and tetanus vaccine;
2.4. line 02 "pertussis revaccination" shall include the number of children who received the 4th vaccination against whooping cough, diphtheria and tetanus at the age of 18 months or 1 year after vaccination was completed;
2.5. line 03 "vaccination against diphtheria - total" includes the number of persons of all age groups vaccinated three times with pertussis, diphtheria and tetanus vaccine or twice vaccinated with diphtheria-tetanus toxoid, antigen-reduced diphtheria-tetanus toxoid and diphtheria toxoid, as well as the number of children , twice vaccinated against whooping cough, diphtheria and tetanus, if more than 12 months have passed since the last vaccination;
2.6. line 04 "including children" shall include the number of children under the age of 17 years inclusive (17 years 11 months 29 days) vaccinated three times with pertussis, diphtheria and tetanus vaccine or twice vaccinated with diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with reduced the content of antigens and diphtheria toxoid, as well as the number of children vaccinated twice with pertussis, diphtheria and tetanus vaccine, if more than 12 months have passed since the last vaccination;
2.7. line 05 "diphtheria revaccination - total" includes the number of children who received diphtheria revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid or antigen-reduced diphtheria-tetanus toxoid, as well as the number of persons aged 18 years and over older, who received revaccination against diphtheria with antigen-reduced diphtheria-tetanus toxoid every 10 years;
2.8. line 06 "including children" includes the number of children up to and including 17 years of age (17 years 11 months 29 days) who received revaccination against diphtheria with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid or antigen-reduced diphtheria-tetanus toxoid ;
2.9. line 07 "vaccination against tetanus - total" includes the number of persons vaccinated three times with pertussis, diphtheria and tetanus vaccine, twice vaccinated with pertussis, diphtheria and tetanus vaccine, if more than 12 months have passed since the last vaccination, and also vaccinated twice with diphtheria-tetanus toxoid, twice vaccinated with diphtheria-tetanus toxoid with a reduced content of antigens and tetanus toxoid. This line also includes the number of persons vaccinated with a double dose (1.0 ml) of tetanus toxoid;
2.10. line 08 "including children" shall include the number of children under 17 years of age inclusive (17 years 11 months 29 days) vaccinated three times with the pertussis, diphtheria and tetanus vaccine, twice vaccinated with the whooping cough, diphtheria and tetanus vaccine, if the last vaccination has passed more than 12 months, as well as twice vaccinated with diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with a reduced content of antigens, tetanus toxoid or single vaccinated with a double dose (1 ml) of tetanus toxoid;
2.11. line 09 "tetanus revaccination - total" includes the number of persons who received revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid, antigen-reduced diphtheria-tetanus toxoid, tetanus toxoid, as well as those previously vaccinated who received tetanus toxoid ( 0.5 ml) for emergency tetanus prophylaxis;
2.12. line 10 "including children" shall include the number of children under 17 years old inclusive (17 years 11 months 29 days) who received revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with a reduced content of antigens, tetanus toxoid, as well as those who received tetanus toxoid (0.5 ml) during emergency prophylaxis;
2.13. Row 11 "Vaccination against poliomyelitis" shall include the number of children who received three doses of polio vaccination;
2.14. line 12 "revaccination against polio" includes the number of children who received the 4th, 5th, 6th vaccination against polio;
2.15. line 13 "vaccination against measles - total" includes the number of persons vaccinated against measles once;
2.16. line 14 "including children" includes the number of children up to and including 17 years of age (17 years 11 months 29 days) who received their first measles vaccine;
2.17. line 15 "revaccinated against measles" includes the number of persons who received a second measles vaccination;
2.18. line 16 "including children" includes the number of children up to and including 17 years of age (17 years 11 months 29 days) who received a second measles vaccine;
2.19. line 17 "vaccination against mumps" includes the number of children from 12 months of age vaccinated against mumps once;
2.20. line 18 "mumps revaccination" includes the number of children who received a second mumps vaccination;
2.21. line 19 "rubella vaccination" includes the number of children from 12 months of age vaccinated against rubella once;
2.22. line 20 "rubella revaccination" includes the number of persons who received a second rubella vaccination;
2.23. line 21 "inoculations against typhoid" shall include the number of persons vaccinated against typhoid;
2.24. line 22 "vaccinations against tuberculosis - total" shall include the total number of children vaccinated against tuberculosis;
2.25. line 23 "including newborns" shall include the number of newborns vaccinated against tuberculosis;
2.26. line 24 "vaccination against viral hepatitis B - total" includes the number of persons vaccinated against viral hepatitis B three times and four times (risk groups);
2.27. line 25 "including children" shall include the number of children under 17 years of age inclusive (17 years 11 months 29 days), three times and four times (risk groups) vaccinated against viral hepatitis B;
2.28. row 26 "vaccinations against viral hepatitis A - total" shall include the number of persons vaccinated against viral hepatitis A;
2.29. line 27 "including children" shall include the number of children up to and including 17 years of age (17 years 11 months 29 days) vaccinated against viral hepatitis A;
2.30. line 28 "vaccination against tularemia" shall include the number of persons vaccinated against tularemia;
2.31. line 29 "tularemia booster" includes the number of persons who received a tularemia booster;
2.32. line 30 "vaccination against brucellosis" shall include the number of persons vaccinated against brucellosis;
2.33. line 31 "brucellosis revaccination" shall include the number of persons who received brucellosis revaccination;

The form of the register of insured persons has been updated, for which additional insurance premiums for a funded pension, the procedure for its provision and the formats for their provision in in electronic format.
The register is submitted by the employer, listing additional insurance contributions to the funded pension of insured persons.
The register is formed separately in relation to the amounts of additional insurance contributions to the funded pension of each insured person (if they are paid).
Clarified presentation order employer of the register of insured persons on hard copy and in electronic form with an enhanced qualified electronic signature.
The update affected the registry form for additional insurance premiums DSV-3. It must be submitted by all organizations listing additional. insurance contributions for employees to the funded part of the pension.
The register is submitted quarterly no later than 20 days after the end of the quarter.
The Register specifies:
The total amount of transferred funds,
Name of the insured person,
The amount of additional contributions transferred,
Employer contributions.
The register is submitted to the FIU no later than 20 days from the date of the end of the quarter during which additional contributions were transferred and contributions were paid.

the form "Register of insured persons for whom additional insurance premiums for a funded pension are transferred and employer's contributions are paid" (form DSV-3), in accordance with Appendix No. 1;
The procedure for submitting a register of insured persons for whom additional insurance premiums for a funded pension are listed and employer contributions are paid, in accordance with Appendix No. 2;
The format for submitting the register of insured persons for whom additional insurance contributions to the funded pension are listed and employer contributions are paid, in electronic form (data format), in accordance with Appendix No. 3.
2. Recognize as invalid:
